VNA Hospice Clinical Liaisons

For in-patient consultations, Cape Cod Healthcare VNA Hospice provides nurse liaisons to help families considering hospice understand the services we provide.

Available seven days a week, these RNs help determine a patient's readiness for hospice and can walk families through the many services VNA Hospice provides, discuss in-home or skilled nursing facility care, and offer guidance to help navigate the financial path of this care.

The goal of these RN liaisons is to help patients and their families make a smooth, seamless transition from curative care at Cape Cod Healthcare's hospitals to comfort care through the Cape Cod Healthcare VNA Hospice.

Certification in Hospice and Palliative Care Nursing, active since 2000.
M.A. in Pastoral Counseling, Gordon-Conwell Theological Seminary, 1996.
B.S. in Nursing, Cornell University, 1975.

Pamela has worked as a hospice nurse, either full or part-time, since 1997. While her initial nursing experience had been in ICU nursing, when the time came to return to work after raising her children, she chose hospice and serves as the VNA Hospice Clinical Liaison for Cape Cod Hospital.

“For me, hospice is about bringing calm, dignity and hope to patients and families who are often in turmoil and don’t know how they are going manage the dying and the death of a loved one. Hospice has given me the privilege of walking this journey with patients and families.”

Diane has worked in Hospice since 1992, first as a Home Health Aide while attending nursing school, then as an RN Case Manager, and currently as a Clinical Liaison for the Cape Cod Healthcare VNA Hospice. While a large part of her nursing career was in maternity, Diane maintained a passion for Hospice nursing and was always drawn back to it.

“It is such a privilege to be present with a patient and their family during such a personal and private time in their lives. I find that to be true for both Maternity and Hospice nursing. In my role as a liaison, I get great personal satisfaction in being able to provide the support and resources families need to ensure their loved one’s comfort at end of life.”

Wilhelm has worked for 10 years as a hospice nurse, following a 13-year career as a medical/surgical nurse. He specializes in geriatric hospice care and has a passion for tending to the needs of elderly patients.
