At the Visiting Nurse Association (VNA) of Cape Cod, our goal is to provide comprehensive home health, hospice services and public health programs that enable individuals - from newborns to the elderly - to receive care in the privacy and comfort of their own homes and support through community services. Our highly trained professionals are dedicated to ensuring a smooth transition from a hospital, nursing home or clinic to home.
Our staff is dedicated to patient care and safety, in collaboration with physicians and healthcare professionals.
Who We Are
Established in 1916, the VNA of Cape Cod is one of the oldest not-for-profit organizations on the Cape. The VNA provides an array of health services from skilled nursing and therapies in the home to community health programs. Changes in the healthcare environment and technology, such as earlier hospital discharges, have led to an increasing need for the services of the VNA. To meet these challenges, we have grown to more than 400 employees, providing more than 200,000 home visits annually.
Available 24 hours a day, 7 days a week, the VNA of Cape Cod cares for patients from Bourne to Provincetown and the Islands, a service area spanning more than 1,000 square miles.
The VNA of Cape Cod is accredited by the Joint Commission for Accreditation of Healthcare Organizations, is Medicare/Medicaid certified, and accepts most private insurance.
The VNA of Cape Cod is a proud member of the Cape & Islands Association of Community Health Agencies, Home & Health Care Association of Massachusetts, Visiting Nurse Association of New England, Inc., and the National Association of Home Care.
What We Do
Whether visiting patients recovering from surgery, teaching patients and families how to manage acute and chronic conditions or providing care and support for a terminal illness, our VNA professionals have earned a reputation for their compassionate attitude, expertise and professionalism. Our skilled team offers a wide range of services that allow patients to safely recover where they feel most comfortable, in their own home.
The increased complexity and advanced technology and treatment options for patient needs upon discharge from the hospital has required VNA staff to be skilled in complicated, high-tech care. Specially trained teams offer intravenous therapy, wound care, ostomy care, pain management, palliative and hospice care, mental health assessments, telemonitoring and maternal and child health. We provide seamless coordination of care through our advanced technologies. In keeping with our mission, we are continuously expanding the scope of our services and raising the quality of care we provide.
