Sports Physicals

Get your athlete ready to play with a sports physical at any of our conveniently located urgent care locations on Cape Cod and in Plymouth.

Sports physicals or pre-participation physical examinations (PPE) are a requirement for participating in most school sports programs and aim to keep your child safe – reducing the risk of preventable issues or injuries.

What to Expect During a Sports Physical

Sports physicals are available by appointment or via walk-in at any of our urgent care locations and require a $75 payment at the time of service.

After arriving for a sports physical:

  • Parents or guardians will complete a brief medical history questionnaire [PDF]
  • The patient will then see a nurse or urgent care technician for basic intake, which includes vital signs, height/weight, visual acuity, medical and surgical history, current medications, allergies, etc.
  • A board-certified provider will then provide an assessment to determine participation clearance

Please bring any vaccination records, lists of ongoing medications, and details about any past injuries or medical conditions.

Patients under the age of 18 will require parental/guardian consent for treatment. This consent can be obtained via telephone if the patient does not present with their parent or guardian.
