Whether you need an annual physical, aren’t feeling well, or are seeking treatment for symptoms, your PCP is your trusted health partner who understands your health history, maintains your medical record, helps manage medications and provides wellness education including recommended screenings. Regular visits with your PCP provide the best prevention and care for your healthcare needs, helping to support long-term wellness.
Patient-Centered Approach
We take a patient-focused, preventive medicine approach that fosters empowerment, education, and advocacy for everyone. We offer different models of primary care through the Medical Affiliates of Cape Cod (MACC), a division of primary and specialty care physicians from Cape Cod Healthcare, from group practices to solo practitioners. Our PCPs at MACC work collaboratively with Cape Cod Healthcare Hospitals, Emergency Centers and our seven Urgent Care Centers to ensure patients have follow-up care after they leave these facilities.

Your care team is led by your PCP and may include highly skilled physician assistants, nurse practitioners, nurses and support staff who coordinate optimal patient care, including referrals to the appropriate medical specialists, when necessary.

Telemedicine
A telemedicine or virtual medical appointment, available through select primary care providers, utilizes smart phone, tablet or computer, to provide health care from a distance. Video visits allow healthcare providers to deliver quality care in the privacy of your own home while connecting with you in a meaningful way. Your physician will help determine what type of appointment is right for you, as well as provide you with the exact steps for access if a virtual appointment is made.
