The Josephine M. Lilly (JML) Care Center is a renovated, 132-bed skilled nursing, long-term care, and short-term rehabilitation facility conveniently located on the Falmouth Hospital campus. Dedicated to providing compassionate, patient-centered care in a welcoming and supportive environment, JML offers personal care and state-of-the-art treatment for patients requiring post-hospital rehabilitation, sub-acute care, long-term care and palliative care services. JML Care Center is comprised of three specialized units: the Penzenance Unit, which focuses on short-term rehabilitation and post-acute recovery, and the Naushon and Nobska Units, which provide long-term care services. Penzenance Unit – Short-Term Rehabilitation Services
The Penzenance Unit is dedicated to short-term rehabilitation and post-acute recovery services. This unit is designed to support patients transitioning from hospital care back to independent living or long-term wellness management.

Naushon and Nobska Units – Long-Term Care Services

JML Nurse (RN & LPN)
Nurses at JML play a meaningful role in helping patients recover, regain independence, and transition through important stages of care. Our nurses provide comprehensive assessments, medication administration, wound care, patient and family education, and collaborate closely with an interdisciplinary team to deliver personalized care plans.

JML Infection Control Preventionist Nurse
At JML Care Center, the Infection Control Preventionist Nurse plays a vital role in ensuring the health and safety of residents, staff, and visitors by preventing, identifying, and managing infections. This includes monitoring for signs of infection, tracking trends, enforcing isolation precautions, and ensuring adherence to cleaning and disinfection protocols. They also develop and implement infection control policies, educate staff on best practices, and collaborate with healthcare teams and public health agencies to respond to outbreaks and report communicable diseases.
JML Physical Therapist/Physical Therapy Assistant
Physical Therapist
This position is responsible for evaluating patients and developing individualized physical therapy treatment plans based on physician referrals, patient conditions, and medical records. Duties include administering therapeutic exercises, functional training, physical modalities, massage, traction and other rehabilitative treatments to improve mobility, strength, and overall function. The Physical Therapist monitors patient progress, adjusts treatment plans as needed, educates patients and families on home care procedures, and collaborates with physicians and healthcare staff to coordinate comprehensive patient care. Additional responsibilities include supervising assistants and aides, maintaining accurate documentation, ensuring confidentiality of resident information, and providing service excellence through compassion, accountability, respect, excellence, and service.
Physical Therapy Assistant
This position is responsible for assisting patients with therapeutic exercises, mobility training, and rehabilitative treatments under the direction of a Physical Therapist. Duties include administering therapeutic modalities such as heat, ultrasound, and electrical stimulation, assisting patients with daily functional activities and mobility, monitoring patient progress, and reporting responses to treatment. The role also involves fitting and training patients on supportive devices, collaborating with the therapy team to coordinate care, supervising aides, and performing clerical and departmental support duties. Candidates must maintain confidentiality of resident information, provide compassionate and professional care, and demonstrate strong teamwork, communication and organizational skills.
JML Social Worker – Skilled Nursing
This position is responsible for assessing patients’ and residents’ psychosocial needs, developing individualized care and discharge plans, and coordinating services to support successful transitions within skilled nursing, long-term care, and community settings. Duties include collaborating with interdisciplinary teams, connecting residents and families with community resources, assisting with advance care planning, completing required documentation and MDS assessments, advocating for resident rights, and maintaining communication with families, healthcare providers, and community agencies. The role requires maintaining confidentiality of resident information, ensuring compliance with state and federal regulations, and consistently providing compassionate, respectful, and professional service. Qualified candidates must possess a Bachelor’s degree in Social Work or a related field, Massachusetts licensure, healthcare social work experience, strong communication and organizational skills, and the ability to work collaboratively with residents, families, staff, and outside agencies.
JML Massachusetts Certified Nursing Assistant (CNA)
Certified Nursing Assistants (CNAs) play a vital role in patient recovery and are essential in supporting RN/LPN staff in delivering quality patient care, particularly during the post-acute phase. In a skilled nursing facility, CNAs assist residents with daily hygiene and personal care needs, monitor for changes in condition and promptly report concerns to nursing staff, support mobility and transfers, and assist with daily activities that promote comfort, safety, and independence. Their contributions are essential to maintaining residents’ quality of life and ensuring continuity of care within the healthcare team.
JML Skilled Nursing Cook/Dietery Aides
Skilled Nursing Cooks
These positions oversees kitchen operations in the absence of the Food Service Director or Executive Chef and is responsible for meal preparation, dietary coordination, food safety, sanitation, and maintaining quality food service standards. Additional duties include supervising kitchen operations, ensuring compliance with resident rights and confidentiality policies, maintaining equipment and work areas, and supporting a safe and efficient food service environment. Candidates should have a high school diploma or equivalent, strong leadership and communication skills, prior food service supervisory experience preferred, and obtain ServSafe and Allergen certifications within six months of hire.
Dietery Aides
This position is responsible for preparing and serving meals in accordance with portion control procedures and resident dietary needs while maintaining cleanliness and sanitation standards within the food service area. Duties include processing diet orders and meal changes, assisting with meal preparation, delivering nourishments, handling food disposal, washing dishes, and supporting kitchen operations as needed. The role requires maintaining confidentiality of resident information, providing excellent customer service, and demonstrating compassion, accountability, respect, excellence, and service in all interactions. Candidates must be able to communicate effectively in English, follow instructions, work independently, maintain a safe and organized work environment, and interact professionally with residents, staff, families, and the public. Physical requirements include the ability to lift up to 50 pounds and perform the physical demands of the position.
