CCHC to award $150,000 in Community Benefits Grants
(Sept. 19, 2011) Cape Cod Healthcare announced today that as part of its Community Benefits program, it will award approximately $150,000 in grants through a competitive Request for Proposal process. CCHC will likely award a maximum of five Strategic Grants of up to $25,000, and a maximum of five Mini Grants of up to $5,000.
Proposed projects must align with one of the following five priorities, as defined in the CCHC Community Benefits Plan for FY12:
- Increase health care access through outreach and leadership initiatives to reduce health disparities and promote health equity.
Create strategic collaborations with existing community health providers and partners
to prevent or delay chronic disease related to asthma, cardiovascular disease, cancer, diabetes or dental health.
- Improve post hospital discharge and follow-up experience of vulnerable patients, particularly elders and those with chronic disease.
- Adopt preventative model for mental health and substance abuse.
- Reduce health risks and unnecessary declines in quality of life through wellness and prevention approaches with attention to particularly vulnerable populations.

Replies will be posted by October 5, 2011.
The deadline for proposals is Friday, October 14, 2011 by 4:00 PM. Applicants should submit an electronic copy of their proposal to email@example.com. In addition two hard copies should be mailed or delivered to Mary Pumphery at CCHC Administrative Office, 88 Lewis Bay Road, Hyannis, MA 02601. Grants will be awarded by December 31, 2011.
