Cape Cod Healthcare Helping Hand Program Helps Patients After a Hospital Stay
Cape Cod Healthcare is pleased to offer a new service designed to assist patients after they return home from a hospital stay. The Helping Hand program is an outreach service that helps people recover and remain healthy at home following a hospitalization.
The CCHC Helping Hand program is offered free of charge by Cape Cod Healthcare. Patients are informed about the service by their nurse or case manager prior to release from the hospital. The goal is to work together with the patient to make sure he or she stays safe and healthy at home.
Within a few days of discharge from the hospital, a pharmacist calls the patient to obtain his or her permission to visit them at home. During the visit, the pharmacist will:
• Review the patient’s prescription and over-the-counter medications,
• Address any concerns the patient has about his or her health and medications,
• Help coordinate a physician follow-up visit and prepare a list of questions to be answered by the physician,
• Continue to support the patient for the next 30 days, and recommend community programs and resources to help the patient stay healthy and remain at home.
Studies have shown that patients who take advantage of the free Helping Hand program are healthier and have fewer readmissions to the hospital.
