Outstanding Achievement Award
Falmouth Hospital Cancer Program received the Outstanding Achievement Award for a three-year period 2011-2014 from the Commission on Cancer. FH was one of only four hospitals in Massachusetts, and 104 nationwide, to receive the distinction. The standards that form the basis of the award criteria are drawn from six areas of program activity: cancer committee leadership, cancer data management, clinical management, research, community outreach, quality improvement.
VNA of Cape Cod Named to HomeCare Elite™
The Visiting Nurse Association (VNA) of Cape Cod announced that it has been named to the 2012 HomeCare Elite™ for the seventh year in a row. The 2012 HomeCare Elite is a compilation of the top-performing home health agencies in the United States. Now in its seventh year, the HomeCare Elite identifies the top 25 percent of agencies and further highlights the top 100 and top 500 agencies overall of the 10,177 reviewed. Winners are ranked by an analysis of publicly available performance measures in quality outcomes, process measure implementation, patient experience (Home Health CAHPS), quality improvement, and financial performance.
Blue Cross Blue Shield “Hospitals of Distinction”
Cape Cod Hospital and Falmouth Hospital have both received the 2012 Blue Cross Blue Shield of Massachusetts “Hospital of Distinction” Award for quality Hip and Knee Replacement. This award recognizes both hospitals as “Blue Distinction Center for Knee and Hip Replacement.” The designation is given to “leading institutions that meet clinically-validated quality standards and deliver better overall outcomes in patient care,” according to BCBS
Falmouth Hospital a Harvard Pilgrim Top Performer
Falmouth Hospital has been named to the 2012 Harvard Pilgrim Hospital Honor Roll. The Honor Roll names those hospitals whose performance was among the top 25 percent of hospitals measured nationally on a set of composite quality metrics. The metrics evaluate clinical process of care, patient experience and patient safety, and are based on data from Centers of Medicare and Medicaid Services (CMS) on Hospital Compare and from The Leapfrog Group
Cape Hospitals Earn National Patient Safety Recognition
Cape Cod Hospital and Falmouth Hospital have earned a national distinction for excellence in patient safety, as documented in national patient safety measures. The two hospitals received an “A,” the highest grade available, in The Leapfrog Group’s patient safety rating program. More than 2,600 hospitals across the United States were issued a Hospital Safety Score, when the results were announced earlier this year. The Hospital Safety Score grades hospitals on how safe they are for patients. Each score, ranging from A to F, is derived from expert analysis of publicly available data on rates of infections, injuries and errors. Falmouth Hospital a Joint Commission Top Performer
Falmouth Hospital has been recognized by The Joint Commission as a 2011 Top Performer on Key Quality Measures™. Falmouth Hospital was cited for its excellence in three key quality measures: heart attack, pneumonia and surgical care. The Joint Commission (TJC), an independent, not-for-profit organization, accredits and certifies more than 19,000 healthcare organizations and programs in the U.S., including Falmouth Hospital and Cape Cod Hospital. TJC recently released its Improving America’s Hospitals: The Joint Commission Annual Report on Quality and Safety 2012. Cape Cod Hospital Ranks Tops in National PCI Mortality Report
Cape Cod Hospital was again ranked in the 90th percentile (top ten percent) among all hospitals nationwide in terms of risk-adjusted mortality for percutaneous coronary intervention (PCI) procedures, according to a database report issued by the American College of Cardiology (ACC). Percutaneous Coronary Interventions (previously called Angioplasty, Percutaneous Transluminal Coronary Angioplasty, or Balloon Angioplasty) encompass a variety of procedures used to treat patients with diseased heart arteries that can result in reduced blood flow, or a heart attack caused by plaque that completely blocks the artery.

Two Joint Commission (JC) surveyors visited Falmouth Hospital in February 2011 for its routine 3-year survey and renewed the hospital's accreditation. The surveyors spent a week at the hospital and gave it a glowing report. In fact, former hospital Chief Operating Officer Sue Wing said it was the best JC report in her 40 years at the facility.

JML Care Center Receives 5-Star Rating from CMS
JML Care Center received a 5-star rating for overall quality from CMS, the Centers for Medicare & Medicaid Services. JML also received 5-star ratings for Health Inspection and RN Staffing. The health rating takes into account the last 3 years of on-site inspections as well as any complaint investigation findings. RN Staffing ratings are based on the amount of time Registered Nurses (RNs) spend with patients. JML was also cited having no deficiencies from incidents or complaints filed for the last three years.

VNA of Cape Cod was recognized as one of the top 500 home care providers nationwide (Top 5%). The Analysis is based on quality of care, process measure implementation and financial performance (public reported info). Of 9,724 Medicare certified home health agencies, the VNA is one of only 338 agencies recognized all six years.

Dr. Zelman Honored as one of America’s Top Doctors®
Richard Zelman, MD, FACC, Director of Interventional Cardiology for CCHC, was recognized by Castle Connolly Medical Ltd. as one of America's Top Doctors®. Congratulations to the Falmouth Hospital Emergency Department physicians and staff for achieving an improvement of 4.4m Mean Score points in Overall Emergency Department Satisfaction on the January 1, 2011 Press Ganey report. This is a statistically significant improvement for Falmouth Hospital, and the result of hard work and dedication to quality improvement. Falmouth Hospital recently received the Defect-free Care Award from the Stroke Collaborative Reaching for Excellence (SCORE), a voluntary statewide quality improvement collaborative administered by the Massachusetts Department of Public Health (DPH), that supports Primary Stroke Service hospitals. Defect-free care is achieved when every patient receives the appropriate care based on clinical guidelines.

CCHC Named Top 10 Health Systems of 2011
The study – the Thomson Reuters Top 10 Health Systems 2011 – was conducted by Thomson Reuters, one of the world’s leading health care information organizations. This annual, quantitative study is based solely on objective, quantitative performance data, independent research and public data sources. This year’s study examined 285 organizations – almost every U.S. health care system with two or more acute care member hospitals.
