• Summary:

  The Diabetes Education Program begins with a one on one visit with a Nurse Educator or Nutritionist to enable us to place you in the educational sessions that would best meet your needs.

  Group sessions are two hours in length and include:
  DE 1 Class: Diabetes Overview ~ Standards of Care
  DE 2 Class: Nutrition Guidelines ~ Exercise Guidelines & Goal Setting

  The Self-Management Program is also offered on an individual basis.
  Appointments and class registration is by physician referral.
