Helping Hands - Your Care Coordinator

helping hand logo and infographicLiving with a complex or chronic health condition can be challenging, but now CCHC Employee Health Plan (EHP) Members have an even greater support system provided through the EHP's Helping Hands Program!

Under this program, care coordination is an approach to optimizing the health and wellness of individuals, often with one or more chronic conditions, through the active support of dedicated healthcare professionals. Eligible Members will receive concierge support services through an assigned Nurse Care Coordinator who will work closely with their Primary Care Physician (PCP) to develop a health and wellness plan tailored to their needs.

This program is designed for Members who could be at risk if their health is not proactively managed and who could benefit from an integrated team of experienced healthcare professionals eager to work together for the well-being our our Members.

Eligible members will receive concierge support services through an assigned Nurse Care Coordinator.

The Nurse Care Coordinators are all registered nurses located in our community with extensive experience, and success, in working with individuals who strive to live their lives to the fullest, but just need a "Helping Hand". The Nurse Care Coordinators will strive to advocate for Members throughout the continuum of care, working with Members, their care givers, family, physicians, and community resources to empower Members to reach their health and wellness goals.

Coordinated Care Program

Care coordination is the concept of healthcare professionals working with patients to ensure that their health needs are being met and that the right provider is delivering the right care at the right time. Through the EHP, care coordination is done through an integrated team approach that always starts with you and your Primary Care Physician. Depending on your particular situation and needs, the team may also include a Specialist you have been referred to, Tufts Health Plan, and/or the EHP's own Helping Hands Nurse Care Coordinators.
