Disease ManagementThe goal of the VNA of Cape Cod disease management program is to assist those living with a chronic disease to learn and understand early warning signs and steps to avoid hospitalization and live independently at home. With a focus on education and self care management, the VNA will develop and implement a best practice based plan of care tailored to meet the specific needs of the patient to inspire quality of life and overall outcomes

The VNA has a certified diabetes educator specializing in the care and education of people with diabetes for VNA patients (home consult visits) and community groups (facilitating education programs). The educator provides diabetes education to clinicians so they are updated on most current diabetes information and collaborates with physicians in care of their patients.
