The VNA of Cape Cod will assist in finding available funding for your patient's home health care needs, whether it be through Medicare, Medicaid (Medical Assistance), the Older Americans Act, the Veterans Administration, Social Services Block Grant programs, community organizations, secondary insurance, workers' compensation, self pay, state programs and managed care.
To qualify for Medicare Home Health Care benefits, the patient must be homebound on admission and during care. The physician determines the need for skilled medical care in the home. We will work with you to prepare a plan for care at home.
- Patient is unable to leave home or,
- Leaving home takes considerable and taxing effort and,
- if patient leaves their home, it must be infrequently, and for short durations
For example, the patient may leave home for a doctor's appointments and may still be considered homebound if they occasionally go to the barber shop, church, beauty shop, or short walk or drive.
