Research has proven that education in diabetes and self-management skills
helps to develop a person’s confidence to live successfully with their disease.
Cape Cod Healthcare offers a multidisciplinary approach to diabetes treatment,
with an emphasis on education, diet, exercise and on-going self-management.
Our Staff The Cape Cod Healthcare diabetes
education team includes registered dietitian/certified diabetes educators and
registered nurse/certified diabetes educators.
Accreditation The American Diabetes Association
recognizes the Cape Cod Healthcare education service as meeting the National
Standards for Diabetes Self- Management Education.
Self Management Program Learning
for Life, the Diabetes Education Program begins with a one on one visit
with a Nurse Educator or Nutritionist to enable us to place you in the
educational sessions that would best meet your needs.
Group sessions are two hours in length and include: • DE 1
Class: Diabetes Overview and Standards of Care • DE 2 Class: Nutrition
Guidelines and Exercise Guidelines & Goal Setting
The Self-management Program is also offered on an individual basis.
Specialized Services Group and individual
sessions are available with the registered dietitian/certified diabetes educator
and registered nurse/certified diabetes educator.

cover diabetes education. Please check with your insurance company before making
