Bourne Health Center Physicians Shift to Convenience Care Model

Cape Cod Healthcare is pleased to announce that its primary care physicians at the Bourne Health Center are converting to a care model that is more effective at treating the whole patient, rather than treating the symptoms of an illness.

The center will transfer its "walk-in care" model to a "convenience care&" model, effective May 7, 2012. Under the convenience care model, medical care at the center will move away from just treating a set of symptoms or an illness, to actively engaging both the patient and care team members in proactive and preventive/wellness care. Convenience care also enhances monitoring of chronic care conditions that need to be periodically re-assessed to maximize overall patient health.

"Convenience care is one of the primary bridges towards assessing and treating the patient's total health care needs," said Miguel Prieto, M.D., a primary care physician at the Medical Affiliates of Cape Cod-Bourne practice, located in the Bourne Health Center. "Often, patients using a walk-in service want to be seen, get treated and move on. In reality, there could be other medical needs that the patient has that if left unattended or assessed, might lead to a worsening of the patient's overall health. The walk-in care concept does not provide essential continuity and flow of information between care team members and the patient. Convenience care does."

All current Bourne Health Center patients will still have access to "walk-in care" services. The only difference is that you must now be established as a patient to access walk-in care going forward.
