FY2014 – FY2016 Community Health Needs Assessment (CHNA)

In September of 2013, the Cape Cod Hospital and Falmouth Hospital Community Health Needs Assessment (CHNA) Report and Implementation Plan for Fiscal Years 2014 – 2016 was released and made available to the public via CCHC’s website: www.capecodhealth.org/communitybenefits

Features of the CHNA Report and Plan include:

  • Data on the health status of Barnstable County residents with accurate comparisons to state and national benchmarks
  • Identified significant health needs and vulnerable populations
  • Community Benefits programs, hospital planning activities and community assets available to address significant health needs

Request for Proposal (RFP) Process

Cape Cod Healthcare Community Benefits offers an annual competitive RFP grants program open to all community organizations with programs aligned with Community Benefits priorities. A formal RFP announcement will be released in the summer of 2016 for the 2017 Annual Strategic Grant cycle. The funding period will be 1/1/2017 through 9/30/2017.

Community Collaborations

CCHC Community Benefits staff play an active role in community coalitions, various regional task force efforts, and health and human service organizations across Barnstable County including leadership participation with the Cape Cod Community Health Area Network (CHNA 27) Steering Committee, the Behavioral Health Provider Coalition of Cape Cod & the Islands, the Barnstable County Regional Substance Abuse Council, and the Barnstable County Human Services Advisory Council.

Community Benefits Reporting

Annual compliance reports are submitted to the Massachusetts Attorney General, the Massachusetts Department of Public Health and the Internal Revenue Service. Additionally, the Staff provides reports to the Community Health Committee, CCHC Leadership, CCHC Board of Trustees, interested agencies and the community at large.
